the form to prove the claim (background report on defects presumably caused by Thalidomide/Contergan, description of the defects caused by Thalidomide/Contergan) (page 6-8),
the waiver in accordance with article 15 of the German Foundation Law (ContStifG) (page 9).

The Foundation (Contergan-Stiftung) only accepts the German forms.
Everything has to be filled out in German or be accompanied by a certified German translation. As suggested in the letter by the Foundation (No 1) you can also state your claim first informally and then later -as a second step- send all the documents and proofs necessary.
